Personnel Requirements — Based on record review, the licensee did not comply with the section cited above as LPA observed that S1 was missing proof of Tuberculosis (TB) clearance and health screening that is not more than one year prior to or seven days after employment which poses a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that several children’s sign in/sign out were missing full authorized representatives’ signatures which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above as S1 was missing the Mandated Reporter Child Care Providers (AB1207) certificate which poses a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that sign in/out sheets did not all have full signatures from authorized representatives which poses a potential health, safety or personal rights risk to persons in care.
Operating without a license — Based on observation and interview, the licensee did not comply with the section cited above as LPA observed a opened gallon of 2% milk in the kitchen fridge and Licensee stated that the 2% milk was served to children due to being unable to find 1% milk at the store which poses a potential health, safety or personal rights risk to persons in care.
