Staff immunization — influenza, pertussis and measles — Based on LPA's review of three staff (LS & S1-S2) which revealed LS & S1-S2 were missing required staff immunization records. This poses/posed a potential health, safety and/or personal rights risk to the children in care.
Personnel Records — Based on the Licensee's statement confirming S2 worked at the facility, however; LS did not furnish records for S2.
