Outdoor Activity Space — Based on LPA observation of wooden fencing near ground in children's play area to be spliting and peeling paint accessable to chidlren. Personnel Records — Based on LPA file reivew, Licensee was not able to provide proof of health screening report for 3 staff which poses a potential Health, Safety or Personal Rights risk to children in care.
Child's Medical Assessments — Based on LPA file reivew of 7 children's files, 6 files did not include the physican report of medical assessment. Licensee failed to ensure medical assessments were completed within 30 calendar days of enrollment which poses a potential Health, Safety or Personal Rights risk to children in care.
Mandated reporter training — Based on LPA review of staff files. All staff files reviewed showed all staff mandated reporter training expired in 2019. Licensee did not ensure completion of renewal trainings of staff which poses a potential Health, Safety or Personal Rights risk to children in care.
