Child's Records — Based on record review, the licensee did not comply with the section cited above in 2 of 5 child files reviewed are missing LIC701 Physician Report's. 1 of 5 child files reviewed are missing LIC700 Identification and Emergency Information and LIC 627 Consent for Emergency Medical Treament which poses a potential health, safety or personal rights risk to persons in care.
