Teacher-Child Ratio — Based on observation and record review, the licensee did not comply with the section cited above in that from 10:23am - 10:27am there was 1 Teacher's Aide supervising 8 children without being directly supervised by a Teacher. In this instance there were 0 teachers visually observing and supervising 8 children. Upon notification, the Director immediately rearranged staff to comply with this requirement. This poses an immediate health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on the observation, the Licensee did not meet the above regulation which poses an immediate Health & Safety Rights risk to the children in care. During the facility tour of the preschool program the LPA observed hazardous items accessible to the children in care including medication located in a staff purse on the counter. Additionally, the LPA observed an excess amount of medication stored in an unlocked cabinet. Items were removed during inspection.

Responsibility for Providing Care and Supervision — Based on the observation, the Licensee did not meet the above regulation which poses an immediate Health and Safety Rights risk to the children in care. During the inspection the LPA heard staff ask, “Who is in there?” The staff proceeds to walk to the restroom directly across the hallway from the classroom and walk away. LPA walked to the restroom and observed a child playing in the water with no other staff present. LPA looked in the classroom and observed the Licensee, Director, and 3-4 staff talking. LPA called the Licensee over to make the same observation. She called a

Teacher-Child Ratio — Based on the observation and file review, the Licensee did not meet the above regulation which poses an immediate Safety risk to the children in care. During the facility tour the Director called S5 to the classroom as the Lead Teacher with one Aide and 18 children. After the staff file review it was determined S5 is not a fully qualified teacher. S5 is missing one core class requirement of Curriculum.

Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above, the preschool classroom had an unlocked drawer, drawer contained 3 screwdrivers, bolts, and a knife. Also observed under the sink was bottle Odo Ban, which is a cleaning agent which poses an immediate health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in that there were 10 total items that stated "call poison control" on their label. These items were located in each of the 3 preschool rooms and were inaccessible to children, however the storage area did not have a key lock or combination lock, which poses/posed a potential health, safety or personal rights risk to persons in care.

Notice to parents that site visit reports are available — Based on record review and interview, the licensee did not comply with the section cited above in that 2 out of 5 children's files (for Child 2 [C2] and Child 4 [C4]) did not have verification of receipt of a licensing report for a report dated 6/17/25. This poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health risk to the children in care. During file review the LPA was unable to review complete immunization records for S2 and S4.

Personnel Requirements — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review the LPA was unable to review a current EMSA approved Pediatric CPR/1st Aid certificate for all staff.

Food Service — Based on the observation, the Licensee did not meet the above regulation which poses a potential Health Rights risk to the children in care. During the facility tour of the kitchen the LPA observed the refrigerator to have sticky spilled liquid, open & uncovered drinks, old food in bags. Director stated the fridge is used for both staff and children’s food. Refrigerator cleaned during inspection.

Drinking water lead testing — Based on record review, the licensee did not comply with the section cited above, lead testing was not completed within the allotted time frame, which poses a potential health, safety or personal rights risk to persons in care.
