Infant Safe Sleep — Based on observation and interview, the licensee did not comply with the section cited above for two out of five play yards which poses a potential health, safety or personal rights risk to persons in care. Two out of three cribs were also obseved to be torn and LPA observed blankets covering the sides of some play yards and blankets inside some. No children were observed to be napping during the inspection.
Mandated reporter training — Based on observation, interview and record reviewm the licensee did not comply with the section cited above for licensee and assistant which poses a potential health, safety or personal rights risk to persons in care. Licensee and assistant have not renewed the Mandated Reporter Training that expired on on 08/19/2025 and 10/17/2023 respectively.
Immunizations — Based on interview and record review, child C2 was missing immunization record which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on interview, and record review], child C1 was missing infant sleep plan LIC 9227 which poses a potential health, safety or personal rights risk to persons in care.
Alterations to Existing Building or Grounds — LPA observed 2 infants and 1 preschooler napping in crib in the off limit master bedroom which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — LPA observed that 2 infants and 1 preschooler was asleep in the master bedroom with the door closed while Licensee was in the living room which poses a potential health, safety or personal rights risk to persons in care.
Notice to parents that site visit reports are available — LPA observed no LIC 9224 with parents' signatures for the type A deficiencies cited on 08/11/2023 in 5 out of 7 children's files which poses a potential risk to the Health, Safety, or Personal Rights of children in care.
