Child's Medical Assessments — Based on interview and record review, Of 8 children files reviewed, 6 children were missing LIC 701, physcian's report. the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — Based on interview and record review, the sign in/out log is documenting "mom" or "dad" as person who took child from the facility.
