Infant Care Center Sleeping Equipment — Based on LPA observation, the licensee did not comply with the section cited above in that LPA observed 1 infant sleeping in a crib with a blanket hanging from the rails and a sound machine next to their head. Additionally, LPA observed 4 infants sleeping on mats with blankets.

101226.3 Observation of the Child (b) Any unusual behavior, any injury or signs of illness requiring assessment and/or administration of first aid by staff shall be reported to the child's authorized representative and recorded in the child's record. Based on interviews and record review, the licensee did not comply with the section cited above in that Child # 1 had a red, swollen eye throughout the day and parents were not informed. This poses a potential risk for health, safety or personal rights risk to persons in care.
Indoor Activity Space for Infants — Based on LPA observation the licensee did not comply with the section cited above in that LPA observed 2 infants sleeping in the classroom rather than the designated nap area which poses a potential health, safety or personal rights risk to persons in care.
