Written directives for drinking water lead testing — Sinks "E", "G" and "H" had a result of greater than 5.5 pbb. This poses a potential health and safety risk to children in care.
Staff immunization requirements — Based on record review, Staff #4 is missing immunizations in measles, pertussis and flu, Staff #8 is missing immunizations in pertussis and flu which poses a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review, Staff #3 and #7 are missing health screening report which poses a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — Licensee failed to report to Licensing an injury that occurred on 08/30/24 involving a daycare child (C1) who sustained an injury and 911 was contacted. This posed a potential risk to the health, safety, and personal rights to persons in care.
Reporting Requirements — In August 2024, a full wall was constructed separating the nap area to the activity area and installed permanent bunk beds for children to nap, however Licensing was not notified of the structural changes. This posed a potential risk to the health, safety and personal rights to children in care.
