Inspection Authority of the Department — Licensee made false statements about the daycare’s hours of operation and licensee’s location and refused LPAs entry to the facility for the purpose of an inspection. This posed an immediate risk to the safety of children in care.

Revocation or Suspension of a License or Registration — Observations and interviews with the licensee and parents revealed that the licensee falsely stated that the facility was closed and that she and the children were not present. Licensee was observed present in the home with children and licensee eventually came out of the home to refuse entry to LPAs. LPAs observed at least 7 children picked up from the facility. This posed an immediate risk to the safety of children in care

Notice to parents that site visit reports are available — Licensee failed to provide type A reports for the following dates to parents of children in care. Dates are 03/18/2024, 04/17/2024 and 05/08/2024. This poses a potential risk Health, Safety Personal Rights risk to children in care.

Infant Safe Sleep — LPA observed an infant asleep in a room with a blankets over and under the infant. This poses an immediate risk to the Health, Safety or Personal Rights to children in care.

Staffing Ratio and Capacity — LPA observed licensee was out of ratio and caring for four preschool children and three infants alone. This poses an immediate risk to the health, safety, and personal rights of children in care.

Staffing Ratio and Capacity — Based on interview with Licensee, the licensee did not comply with the section cited above by having seven (7) children in care with no assistant, which poses a immediate health, safety or personal rights risk to persons in care.

Staffing Ratio and Capacity — Based on LPAs observation and Licensee's statement, S1 was left alone supervising seven children: four infants and three preschool aged children. This poses an immediate risk to the health, safety, and personal rights of children in care.

Inspection Authority of the Department — Based on interview, the licensee did not comply with the section cited above by not granting LPA entrance into the home to conduct a 3 year required annual inspection, which poses a potential health, safety or personal rights risk to persons in care.

Immunizations — C1, C2 & C3 did not have immunization records present. This poses a potential risk to the health and safety of children in care.

Infant Safe Sleep — Licensee does not have infant sleep logs for infants in care.

Family home — home free of safety hazards — Wood decking on the back of the home has worn and splintered wooden planks and carpet in the playroom has debris and stains. These conditions pose a potential risk to the health and safety of children in care.

Family home — admission procedures and parent rights — 3 children in care did not have LIC995A signed by parents and available in the children's files. This poses a potential risk to the health, safety and personal rights of children in care.

Staff immunization — influenza, pertussis and measles — H1 does not have immunization records on file. This poses a potential risk to the health and safety of children in care.

Family home — preventive health training, including pediatric CPR and first aid — Licensee and Helper could not produce their CPR training certificate. This poses a potential risk to the health and safety of children in care.

Family home — home free of safety hazards — Child 10 did not have a file. This poses a potential risk Health, Safety Personal Rights risk to children in care.

Staff immunization — influenza, pertussis and measles — LPAs observed that S1 does not have immunizations in Measles, Pertussis, and Flu. This poses a potential risk to the health, safety or personal rights to children in care.

Infant Safe Sleep — LPAs observed that licensee does not have documentation of 15 minutes sleep check for infants (C1, C2, C4, & C12). This poses a potential risk to the health, safety, or personal rights to children in care.

Mandated reporter training — LPAs observed that Licensee and S1 do not have a current Mandated Reporter Training on file. This poses a potential risk to the health, safety or personal rights to children in care.

Child's Records — Based on LPAs observation and review of records, all of the children in care have incomplete files.

Infant Safe Sleep — Based on LPAs observation and review of records, C4 & C12 are missing the Individual Sleeping Plan (LIC 9227) on file.

Family home — home free of safety hazards — Based on interview, the licensee did not comply with the section cited above. Licensee stated she conducted her last drill approximately three months, however, does not have documentation, which poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on interview, the licensee did not comply with the section cited above. Licensee stated that she has not documented the 15 minute checks for infants(C1, C2 and C4),which poses/posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review,the licensee did not comply with the section cited above. Licensee and S1 do not have a current Mandated Reporter Training on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Licensee and S1 do not have current Pediatric CPR/FIRST Aid, which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Records — Based on observation, the licensee did not comply with the section cited above. LPA's were unable to review six children's files, which poses/posed a potential health, safety or personal rights risk to persons in care.
