Family home — home free of safety hazards — Based on observation service tag is dated with last inspection on July 22, 2024, the licensee did not comply with the section cited above in [1] out of [1] fire extinguisher not up to State Fire Marshall standards which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Staff #1 did not have proper immunization's present at the facility, records against Tuberculosis, influenza, pertussis, and measles was not observed or reviewed. Per Staff #1, needs to find immunization records.
Infant Safe Sleep — Staff #1 stated infant does not have a 15 min sleep log and only physically checks on infants.
102416.2(a) Reporting Requirements (a)The licensee shall report the following information the Department by telephone or fax within the Department's next business day and during normal working hours (8am to 5pm). The requirement was not met as evidenced by: based on interviews conducted with Licensee admitted that she did not report incident that occurred on 07/14/24 and police came to her home 07/16/24. Licensee stated she did not report because she did not that she had to. Per Licensee, staff is qualified but but paperwork is in the garage. This poses a potential risk to the health and safety of children in care.
