(1) No child(ren) shall be left without the supervision of a teacher at any time. This requirement was not met by... Teacher failed to provide the required level of supervision. A child was left unsupervised on the playground. This lack of supervision poses an immediate health, safety, and personal rights risk to children in care. Children shall be supervised at all times, including continuous visual observation.
Personal Rights — Based on interview and record review, the licensee did not comply with the section cited above by having a self-reported incident on 27 March 2026 where a child was handled roughly and scratched by a teacher, which poses an immediate health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision — Based on interview and record review, the licensee did not comply with the section cited above by having a self-reported incident on 7 April 2026 where a child left the facility and walked to the sidewalk of the nearby street before being returned to facility where the total time they were without supervision was approximately 2 minutes, which poses an immediate health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above by having a can of bug killer stored in an inaccessible part of facility, but not stored behind a lock as per licensing regulations which poses an immediate health, safety or personal rights risk to persons in care.
