Responsibility for Providing Care and Supervision — based on interviews and observation child #1 singularly eloped and was not observed by any staff which poses an immediate health, safety, or personal rights risk to persons in care.
Limitations on Capacity and Ambulatory Status — During a cite visit conducted at the facility on 6/4/24, LPA toured the facility with the assistance of the Director / Marisol Nepomuceno at 1:30PM and observed 77 children with 6 staff present in the preschool program. The facility is licensed for a maximum capacity of 70 children.

Teacher-Child Ratio — Based on observation, record review, and interview, LPA observed 79 preschoolers during inspection. The maximum capaicty is 70 children, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Daily Inspection for Illness — Based on interviews and disclosures made, it was determined that the facility failed to conduct a daily inspection of Child #1 when dropped off at the facility in which the facility was unable to determine of Child #1 sustained in injury while in care or prior to arriving to the facility which poses a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision — Based on interviews and disclosures made, it was determined there was a lapse in care and supervision in which the facility was unable to determine of Child #1 sustained in injury while in care or prior to arriving to the facility which poses a potential health, safety or personal rights risk to persons in care.
Notice to parents that site visit reports are available — During the interview with the Director, it was disclosed that the Type A citations issued on 5/16/24 were not posted at the facility, as required for 30 days.
