Criminal background check — Based on interviews, the licensee did not comply with the section cited above in that adult #1 did not have a criminal record clearance and has been employed at the daycare cleaning but did not remember what day and had contact with children on 03/19/2026 , which poses an immediate health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on interviews, the Director did not ensure children were adequately supervised by leaving children unattended on multiple occasions while using the restroom, which poses an immediate health, safety or personal rights risk to persons in care.

Plan of Operation — Based on interviews and record review, the licensee did not ensure the facility operated in accordance with the approved Plan of Operation. The Director suspended C1 on 03/19/2026; however, the Parent Handbook and Plan of Operation did not contain a suspension policy at the time of the suspension, which poses a potential health and safety risk to children in care.

Admission Agreements — Based on interviews, messaging records, and document review, the licensee did not ensure the facility operated in accordance with the terms and conditions set forth in the admission agreement. The Director closed the facility with less than 24 hours' notice based on a "two child minimum" requirement not included in the admission agreement, which poses a potential health and safety risk to children in care.
