Notice to parents that site visit reports are available — Based on observations, records review and interview, the licensee did not comply with the section cited above.LPA observed only 5 signed receipts, seven were missing . Per licensee sha has only 5 parents sign , she still need 7 parents signatures, which poses/posed a potential health, safety or personal rights risk to persons in care.

Licensing staff observed latch under teh sink was not working and cabinet was accessible to the children with Clorox spray inside, which poses/posed a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on observations, records review and interview ,the licensee did not comply with the section cited above. # out of 12 were missing immunizations . Licensee was able to obtain 2 during the visit , she still missing one child`s immunization,which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on observations , records review and interview ,the licensee did not comply with the section cited above. Licensee did not have a file for child #13 ,which poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on records review and interview , the licensee did not comply with the section cited above. 3 out of 12 were missing medical consent form, which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on staff file review, licensee's assistants, Vardanush Avetisyan and Ani Yeghiazaryan did not have proof of immunization against measles, pertussis and influenza on file. This poses a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on 15children file review, 5 out of 15 children file was not available today for review . This poses a potential health, safety or personal rights risk to persons in care.

(c)(1) A licensed child day care facility shall provide to the parents or guardians of each child receiving services in the facility copies of any licensing report that documents any Type A citation , etc... This requirment is not met as evidence by... Based on 15 children files review, licensee was not able to provide LPA Acknowledgment Receipt of Licensing Report LIC 9224 for 12/03/24. This poses a potential health, safety and personal right risk to the children in care.
