During the facility file review, the LPA was unable to review a current roster. Three children present were not on the roster. Licensee stated she was working on paperwork.

Staff immunization — influenza, pertussis and measles — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA was unable to review immunizations records for S2. Licensee stated she was unaware S2 was required to have immunizations.
