Reporting Requirements — Based on interview the facility did not follow reporting requirments per Title 22. staff were notified by the parent on two separate occasions regarding marks on C1 by parent. Staff did not provide C1’s parent to Community Care Licensing which poses a potential health and safety risk to children in care.
Outdoor Activity Space for Infants — Based on observation and interview , the licensee did not comply with the section cited above. Per Director the infant program has been using the preschool yard for a few years (approximate date was not provided) without prior approval from CCLD which poses/posed a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision for Infants — Based on record review, the licensee did not comply with the section cited above in 3 out of 3 children's files which did not contain the sleep log which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in 2 out of 3 children's files which did not contain the physician's report which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care Food Service — Based on record review , the licensee did not comply with the section cited above in 2 out of 3 files which did not contain an individual feeding plan which poses/posed a potential health, safety or personal rights risk to persons in care.
Indoor Activity Space for Infants — Based on observation and interview the licensee did not comply with the section cited above. LPAs observed preschool children entering the infant classroom to access the outdoor play yard. Director confirmed this is the only way to access the outdoor play yard which poses/posed a potential health, safety or personal rights risk to persons in care.
Indoor Activity Space for Infants — Based on observation and interview the licensee did not comply with the section cited above. LPAs observe that the napping area was not physically separate from activity space. LPAs observed infants being provided care and activities in the napping area which poses/posed a potential health, safety or personal rights risk to persons in care.
