Family home — children supervised at all times — At 9:30 AM, upon arrival, the LPA observed that the Licensee was not present at the facility. The Licensee arrived at 11:00 AM, only after staff notified them that the LPA was at the facility conducting a cite visit. Based on interviews conducted, statements corroborate that the Licensee’s absences often exceed 20% of the hours the facility is providing care each day. Specifically, it was discovered that on May 22, 2025, the Licensee left the facility at approximately 1:30 PM and did not return until around 9:30 PM. This poses an immediate health, safety, and personal rights risk to children in care.
