Staffing Ratio and Capacity — Based on observation, interview and record review, the licensee did not comply with the section cited above. There were 5 infants (under 24 months) present during today's visit, which poses an immediate health, safety or personal rights risk to persons in care.

Family home — children supervised at all times — Based on observation and interview, the licensee did not comply with the above cited regulation. The licensee’s spouse/assistant disclosed that the licensee had flown out of the country to visit her father and would be absent from the family child care home for approximately 30 days. Although the assistant was supervising the children, the licensee's prolonged 30-day absence does not meet the requirement that the licensee be present and actively involved in the daily operation of the home, which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on the record review and interview, the licensee did not comply with the above-cited regulation. The licensee’s assistant/spouse did not have a valid Mandated Reporter Training certificate and had not renewed the required training. This does not meet the requirement for all staff to maintain current mandated reporter certification, which poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on observation, interview and record review, the licensee did not comply with the section cited above. LPA observed an accessible baby bouncer in the day care area, which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, interview and record review, the licensee did not comply with the section cited above. LPA observed children sleeping in cribs with loose blankets, which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, interview and record review, the licensee did not comply with the section cited above. LPA observed a mobile toy hanging over one crib . LPA also observed blankets hanging on the outside of each crib, which poses a potential health, safety or personal rights risk to persons in care.
