Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in that staff listed on staff review form LIC 859 had expired mandated reporter certificates which posed a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on LPA interviews and records review, the facility did not comply with Title 22 regulation-personal rights. A child was handled in a rough manner when pulled by the arm and holding them back by the shoulders which posed a potential health, safety or personal rights risk to children in care.
101223 (a)(3) The licensee shall ensure... personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse... interference with functions...This requirment was not met as evidenced by: Based on LPAs interviews, staff taunted and threatened a child due to the child refusing to nap and not getting along with others. In addition, Staff repeatedly slammed their fists on a desk and repeatedly stated to call the child’s authorized representative.
Indoor Activity Space — Based on observation, the licensee did not comply with the section cited above which poses a potential personal rights risk to persons in care. Upon inspection, LPAs and LPM observed carpet floors that contained dirt build up.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above which poses a potential health and safety risk to persons in care. S3, S4, and S7's mandated reporter trainings have expired.
Staff immunization requirements — Based record review, the licensee did not comply with the section cited above which poses a potential health and safety risk to persons in care. S4 is missing MMR. S6 is missing MMR and Tdap.
Immunizations — Based on record review, the licensee did not comply with the section cited above which poses a potential health and safety risk to persons in care. C2's immunization records are not complete and have not been updated.
