Limitations on Capacity 101161(a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. — Based on record review, interview, and observation, LPA observed 9 preschool children (3 and 4 year olds), in the Kindergarten classroom. This classroom is part of the School Age license, which poses an immediate health and safety risk to persons in care.

Criminal Record Clearance — Based on records reviewed and interviews conducted the facility did not ensure all staff have clearances transferred and associated to the facility. This is an immediate risk to the health and safety of children in care.

101238.2(d)(2) Outdoor Activity Space (d) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris... that pose a... hazard. This was not met as evidenced by: A box/carton containing multipe razor blades was discovered by children in care, resulting in two injuries. Staff stated they conducted a safety check of outdoor playspace prior to incident but did not observe any hazards. This is an immediate health safety and personal rights risk to children in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation, the licensee did not comply with the section cited above as LPA observed that the rock wall was in disrepair as a few of the handholds are broken and have sharp edges which poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above as LPA observed that S6 was missing proof of current Mandated Reporter Child Care Providers Training (AB1207) which poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S2, S3, S5, and S6 were missing proof of Tuberculosis (TB) clearance and S3, S5, and S6 were missing proof of the health screening which poses a potential health, safety or personal rights risk to persons in care.

Child's Records — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C9 was missing proof of the LIC627 while C7 and C8's LIC627s were not completed correctly which poses a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on record review, the licensee did not comply with the section cited above as LPA observed that C6 was missing proof of updated immunizations which poses a potential health, safety or personal rights risk to persons in care.

Notice to parents that site visit reports are available — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 through C10 were missing proof of the LIC9224 dated for 10/23/25 while C2, C3, C5, C6, C7, C8, and C9 were missing proof of the LIC9224 dated for 02/26/25 which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that: S2, S4, S5, and S6 were missing proof of the measles immunization; S4, S5, and S6 were missing proof of the pertussis immunization; and S5 and S6 were missing proof of the influenza immunization/declination which poses a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based record (video) review) Facility did not comply with the section cited above in according dignity in their (children's) personal relationships with staff and other persons. This is a potential risk to the health and safety of children in care.

101221(C) Reporting Requirements (c) The licensee shall notify the Department in writing of his/her intent prior to making any structural changes that reduce the total amount of indoor or outdoor activity space... This was not met as evidenced by: Facility made alterations and closed/prevented use of outdoor activity space for an undisclosed period of time without reporting closure or alternatives for outdoor activity space to Licensing as required. This a potential health, safety or personal rights risk to persons in care.
