Responsibility for Providing Care and Supervision — a self report incident, and staffs' and child's interviews that S1 left 3 year old child unattended in the hallway during transitioning from one room to another for less than a minute. This is an immediate risk to the health and safety of children in care. This is a repeat same violation within 12 months. An immediate civil penalty of 1000.00 was assessed today.

Responsibility for Providing Care and Supervision — Based on self-report UIR and staff interview conducted on 4/12/24, C1 was left alone without any adult supervision. This pose an immediate risk to the health of the children in care. Based on interviews and statements from Director and HR, S3 failed to supervise a child from Pre/K to T/K room. This poses an immediate risk to the health and safety of the children in care..

Disaster and Mass Casualty Plan — Based on observation and record review, the licensee did not comply with the section cited above in one count out of total of one count lacking the log for fire drill which poses a potential health, and safety risk to persons in care.

Personnel Requirements — Based on observation and record review, the licensee did not comply with the section cited above in 7 counts out of total of eight counts lacking the documentation of certificates for CPR and 1st aid for staff which poses a potential health, and safety risk to persons in care. One staff had the current CPR and 1st aid but not being present during the entire operation hours.

Personnel Requirements — Based on observation and record review, the licensee did not comply with the section cited above in one count out of total of eight counts lacking the health screening document including a test for TB for staff # 7 which poses a potential health, and safety risk to persons in care.

Teacher-Child Ratio — Based on a complaint report, interviewing 8 staff, reviewing name to face sheet dated 6/17/2026 that one staff member was left alone with 20 preschool children from 8:05 to 8:37am. Although S2 was present in the classroom from 8:00am to 8:15 am, S2 had not clocked in, observing a school project, and not supervision children. Additionally, S4 was entering and exiting the classroom. There is no record of signing S4 on the name to face sheet. Therefore, the facility failed to maintain the required preschool teacher child ratio of 1:12 between 8:05 to 8:37 am on 6/17/2026. This is a potential risk to the health and safety of children in care.

Reporting Requirements — Today's observation that the change of director was not reported within 10 days. This is a potential risk to the health and safety of children in care.

Personnel Requirements — Based on reviewing 7 staffs' flies, the licensee did not comply with the section cited above in 7 out of 7 files missing current CPR and 1st aid Certificates which poses a potential health, safety or personal rights risk to persons in care.
