Infants up to 12 months of age who are sleeping in a position other than on their back if the infant’s individual infant sleeping plan LIC 9227 (3/20) does not have Section C completed, staff shall return the infant to their back for sleeping. — reviewing C1’s LIC 9227 which Section C was not filled out completely regarding C1's sleeping position. Upon the occurrence, during the operation of the childcare center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. The above requirement was not met as evidenced by during the course of investigation an alleged incident was not reported to licensing office as it was required to.
