Infants up to 12 months of age who are sleeping in a position other than on their back if the infant’s individual infant sleeping plan LIC 9227 (3/20) does not have Section C completed, staff shall return the infant to their back for sleeping. — reviewing C1’s LIC 9227 which Section C was not filled out completely regarding C1's sleeping position. This is a potential hazard to the health and safety of children in care.
Upon the occurrence, during the operation of the childcare center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containg the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. The above requirement was not met as evidenced by during the course of investigation an alleged incident was not reported to licensing office as it was required to. This is a potential hazard to the health and safety of children in care.
Personnel Requirements — Based on reviewing 5 staffs' flies, the licensee did not comply with the section cited above in 5 out of 5 files missing current CPR and 1st aid Certificates which poses a potential health, safety or personal rights risk to persons in care.
Infant Care Food Service — Based on observation, the licensee did not comply with the section cited above in 2 counts out of 3 counts missing the labels with names and current dated for Child # 4, and Child # 5 which poses a potential health, and safety risk to persons in care.
Responsibility for Providing Care and Supervision for Infants — Based on observation and record review, the licensee did not comply with the section cited above in 10 counts out of 10 toddlers' files missing sleeping log for every 15 minutes check from Monday June 2nd to now which posed a potential health, and safety risk to persons in care.
Personnel Requirements — Based on observation and record review, the licensee did not comply with the section cited above in one count out of 7 staffs' files that staff # 3 was missing LIC 503 including a test for tuberculosis which posed a potential health, and safety risk to persons in care.
