(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. Based on interviews and records reviewed, facility staff failed to provide proper care and supervision for a child that remained in the hallway for an undetermined duration while transitioning from one classroom to another classroom. This poses an immediate health, safety and personal rights risk to children in care.

Upon the occurrence, during the operation of the...center of any of the events... a report shall be made to the Department by telephone or fax within...next working day and during its normal business hours...a written report...shall be submitted...within seven days following the occurrence of such event.
