(b) The licensee may use teacher aides in a teacher-child ratio of one teacher and one aide for every 15 children in attendance. This was not met as evidenced by: Based on LPA observation, S1 (teacher) and S2 (aide) were supervising 20 children from approximatly 10:15am to 10:45am which poses a potential risk to the health, safety, and personal rights of persons in care.
Observation of the Child — Based on record review, the facility did not possess record of incident reports for all children involved in 9 separate incidents that occured between 11/11/25 - 1/23/26 which poses a potential risk to the health, safety, or personal rights of children in care.
Disaster and Mass Casualty Plan — Based on record review, the facility did not perform a disaster within the last 6 months which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the staff files for staff 2 through staff 5 did not contain current mandated reporter training certificates which poses a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review, the staff file for staff 1 did not contain complete LIC503 Health Screenings which poses a potential health, safety or personal rights risk to persons in care.
Child's Medical Assessments — Based on record review, the facility did not possess a complete physician's report for child 2 which posed a potential health, safety or personal rights risk to persons in care.
Teacher-Child Ratio — Based on the LPA's interview with Center Director Alicia Wright’s own admission on 06/11/25, it was corroborated that the facility operated out of ratio on a specified date resulting in one teacher supervising more than 12 awake children for approximately 65 minutes. This posed a potential health and safety risk to the children in care.
Personal Rights — Based on interview and record review C1, a child with known lactose intolerance was served milk or foods containing dairy on more than one occasion, which which poses a potential personal rights or Health and Safety risk to children in care.
