Food Service — Interview with the Director stating the child was present during lunch hours but was not given a lunch.
Reporting Requirements — LPA Katrdzhyan discovered that the facility failed to report an incident to CCL, which took place several months ago (date unknown), involving Child 1 and Staff 1. During an interview with the Director / Cheryl Gartsman, it was confirmed that an Unusual Incident/Injury Report (LIC 624) was not submitted to CCL, per Title 22 Reporting Requirements, Section 101212. This poses a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — On 1/25/24, staff observed rodent droppings in the kitchen, near the refrigerator. The Director also observed rodent droppings in the kitchen on 1/29/24.
