Infant Safe Sleep — Based on observation and interview, the licensee did not comply with the section cited above in that there is not a crib or play yard for the infant in care and assistant stated the infant sleeps on the sofa or they hold the infant which poses an immediate health, safety or personal rights risk to persons in care.
Criminal background check — Based on observation and record review, the licensee did not comply with the section cited above in that assistant (A1) did not have a criminal record clearance, was interacting and watching children and stated she helps out occasionally which poses an immediate health, safety or personal rights risk to persons in care.
Family home — preventive health training, including pediatric CPR and first aid — Based on observation, record review, and interview the licensee did not comply with the section cited above in that the two (2) assistants present during the inspection and alone with children, did not have a valid CPR and First Aid card available for review which poses an immediate health, safety or personal rights risk to persons in care.

Personnel Records — Based on interview and record review, the licensee did not comply with the section cited above in that personnel files were not available for licensee, and two (2) assistants who were present with children which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on interview and record review, the licensee did not comply with the section cited above in that record of immunization's for licensee and two (2) assistants were not available which poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on observation, interview, and record review, the licensee did not comply with the section cited above in that five (5) out of seven (7) children present had incomplete files, missing forms or no forms at all which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, interview, record review, the licensee did not comply with the section cited above in that the one (1) infant in care did not have an LIC9227 form in the file, nor was licensee aware of the form which poses/posed a potential health, safety or personal rights risk to persons in care.
