Plan of Operation — Based on statements provided by parents and staff, and a review of the Parent Handbook which confirmed the facility did not comply with it's own plan of operation. This poses/posed a potential health, safety, and/or personal rights risk to the children in care.
Sign In and Sign Out — Based on LPA's review of the sign in/out procedure which revealed that multiple child were not signed out by their authorized representative. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Criminal Record Clearance — Based on a review of Department records which revealed S1's criminal record clearance was not associated to the facility license. This posed/poses a potential health, safety and/or personal rights risk to the children in care.
Staff immunization requirements — Based on staff (S1-S19 & CD) records reviewed which revealed S1 was missing proof of immunity against Measles. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on staff (S1-S19 & CD) records reviewed which revealed S11's record did not contain a current AB 1207 Mandated Reporter Training certificate. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on staff (S1-S19 & CD) records reviewed which revealed S1 was missing LIC 503 & 501. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
