Documentation of completed Mandated Reporter Training Certirficates were not on file for S3, S4, and S6.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in count out of 3 persons which poses a potential health or safety risk to persons in care. Documentation of a health screening report including tuberculosis test was not on file for S3, S4, and S6.
Personnel Records — Based on record review, the licensee did not comply with the section cited above in count out of 2 persons which poses a potential health, safety or personal rights risk to persons in care. Documentation of criminal record clearance for S3 and S6 was not on file during record review. During the visit, LPA obtained completed LIC 9182 forms from S3 and S6.
