LPA & Office Services Supervisor observed three area rugs in Room 22 that appeared soiled and required deep cleaning.

LPA and Office Services Supervisor observed that the medications of two children (C1 & C2) were not in their original containers and a child's medication (C3) was expired.
Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 3 out of 10 staff files which poses a potential health, safety or personal rights risk to persons in care. Staff (S1, S2, & S3) do not have their proof of immunizations in their files.
Teacher Qualifications and Duties — Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA and Office Services Supervisor observed that Staff (S1) is used as a teacher, however based on review of their transcripts, S1 is missing Child Development and Child/Family/Community classes.
