Personal Rights — Based on interview, record review, and observation, licensee violated Child #1's personal rights by allowing them to use non age appropriate play equipment, resulting in injury needing medical attention. This was an immediate risk to the health, safety, and personal rights of child in care.

Child's Medical Assessments — Based on record review and interview, Licensee did not ensure that Child #3 had a Physician's Report completed as part of a Plan of Correction. This poses a potential risk to the health, safety, and personal rights of children in care.

Drinking water lead testing — Based on observation and record review, the licensee did not comply with the section cited above as they have not tested their water outlets (used for drinking and food prep) for lead exceedance which poses a potential health, safety or personal rights risk to persons in care.

Personnel Records — Based on observation and record review, the licensee did not comply with the section cited above as both Staff #1 and #2 are missing numberous forms required, which poses a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above as 4 of 5 Children Files, Child #1-3, & #5, did not have written medical assessment, which poses a potential health, safety or personal rights risk to persons in care.

Immunizations — This was assessed as a citation in error. All children files reviewed had immunization records.

Personal Rights — Based on record review, the licensee did not comply with the section cited above as 5 of 5 Chldren Files were missing signed LIC613A forms, which poses a potential health, safety or personal rights risk to persons in care.
