Criminal Record Clearance — This report is being amended to correct the code Based on record review, the licensee did not comply with the section cited above in that Brenda Arias De Parada was not associated to this facility and was hired 1/15/24 which poses a potential health, safety or personal rights risk to persons in care.
Child Abuse Central Index — This is an amended report to correct the regulation code. Based on record review, the licensee did not comply with the section cited above in that although Brenda Arias De Parada did have finger print clearance for Assisted Caregiver Services, she did not have her finger prints checked through the child abuse cental index system which poses a potential health, safety or personal rights risk to persons in care.
