Vine Learning Center #2, The

Responsibility for Providing Care and Supervision — Based on staff interviews, and review of camera footage an infant was left on top of a boppy pillow on thefloor. A blanket was left above the infant’s head and due to lack of supervision, the infant was able to grab the blanket and dragged it over the infant’s head.

Personal Rights — Based on staff interviews, and review of camera footage, it was observed that a parent walked into the classroom and found an infant's head covered with a blanket. Video footage reviewed also revealed that the infants head was covered for approximately 45 seconds.

Personal Rights — Based on information rec'd via interviews and document provided today, Staff #1 was observed to grab and sat a child hard on the floor and yelled at the child.

Personal Rights — The squeezing of infant #1's face, as shown in the video provided, which was described to continue to occur from 08/02/2023 to 08/07/2023. It is an indicative of an inflicted injury bruise due to forceful and inappropriate handling of an infant. This poses an immediate health and safety risk to clients in care.

Child's Records — Based on interviews and record review, there was a unenrolled school-age child (C1) present in the care of the Director office, which poses an potential Health, Safety, and/or Personal rights risk to persons in care.

Limitations on Capacity and Ambulatory Status — Based on interviews and record review, LPAs observed an unenrolled school-aged child (C1) present in the breakroom being supervised by the Director, as the facility is licensed to care for infants ages 6wks to 2 yrs old only, which poses an potential Health, Safety, and/or Personal rights risk to persons in care.

Alterations to Existing Buildings or New Facilities — Based on interview and record review, the licensee failed report to the Department, plans for construction at the facility, which poses a potential health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — Based on observation and interviews, the facility was operating out of ratio on 3/18/26 when S1 was alone in a classroom with 19 children, which poses a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on LPA observation on 3/26/26, there were construction materials -- nails and electrical cords -- at the entrance/exit of the Toddler 2 classroom, as well as in proximity to the facility entry gate, which poses a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on interview and record review, site director failed to report an unusual incident via phone or fax to the department within the department's next working day. Incident that ocurred on 2/20/2025 was not reported to the department until 2/24/2025.

Staff-Infant Ratio — Based on LPA's observation, LPA observed Toddler #1 classroom to be out of ratio at 12:35PM. Staff #2 was observed supervising 19 napping toddlers.

Criminal Record Clearance — Based on record review, the licensee did not comply with the section cited above. Substitute Mala Hemmati's fingerprint clearance is not associated to the facility. This poses a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on interview and record review, the licensee did not comply with the section cited above. Staff Megan Gordon did not have the required immunization record on file - Pertussis, Measles and influenza. This poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above. Three staff were missing a copy of Physician's Report - Megan Gordon, Roseanna Necochea & Isabella Lepro. This poses a potential health, safety or personal rights risk to persons in care.

Staff-Infant Ratio — Based on observation, the licensee did not comply with the section cited above in the Infant Room #2 with 14 infants with 1 teacher and 3 aides. Fully qualified teacher was on break. This poses a potential health, safety or personal rights risk to persons in care.
