Personal Rights — Based on documentation and interviews, it was determined that that a staff member recorded and posted the child in an inappropriate manner and without permission. In a separate incident, a child was left without proper clothing, this poses an immediate health, safety or personal rights risk of children in care.

Personal Rights — Based on interviews, video of incident watched and documents gathered it was determined that C1's personal rights were violated by S1 when she physically handled C1 inappropriately.

Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above. LPA observed a standing cleaning water in a bucket in the Monkey's room accessible to day care children during a tour of the facility. This poses an immediate health, safety or personal rights risk to persons in care.

During tour of the classroom, LPA observed canisters of Lysol and and containers of Lysol wipes kept in unlatched cabinet in the Monkeys room that are accessible to day care children. This poses an immediate health, safety or personal rights risk to persons in care.

Admission Policies — Based on interviews and documentation review, the Site Director states that there is no written sanitation policy or procedure or that reusable cups require water only in the parent handbook, which poses a potential risk to the Health, Safety, or Personal rights to persons in care.

Reporting Requirements — Based on documentation and interviews, it was determined that a child was left without proper clothing because of a potty incident, and child's parents were not contacted to make aware of situation. This poses a potential health, safety or personal rights risk of children in care.

Responsibility for Providing Care and Supervision — Based on interviews and documentation reviewed today, a preschool child was left in the playground area unattended and without visual supervision for approximately 6 minutes on 2/21/2025. This is a repeat violation.

Personnel Requirements — Based on record review staff Jessica Olachea (substitute) did not have a copy of the health screening on file. This is a repeat violation.

Teacher-Child Ratio — Based on observation during tour of the facility, Monkeys Classroom was observed with 17 preschool children, with 1 teacher and 1 aide.

Health-Related Services — Based on observation, the licensee did not comply with the section cited above. LPA observed an expired Epi-pen stored in the Butterflies room. This posesd a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above. Dirty mops and brooms were observed accessible to children in the Monkeys, Ladybugs, Bumble bees and Butterflies rooms. This poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above. Staff Angie Gilmore was missing a copy of Health screening on file. This poses a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on information gathered and staff interviews during the investigation, it is determined that teacher S1 did not provided care toward child C1, which is a potential risk to the health, safety, or personal rights of children in care.
