Family home — safe toys and play equipment — During the inspection of the backyard, LPA observed a plastic play structure that has a broken step on the bottom that is wide enough for a child's foot to get caught. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — During review of records, it was discovered the the Licensee did not have any required licensing forms on file for any of 6 children that were present during the inspection. Licensee stated that she provided the parents with new forms to complete as a lot of the families had moved, however per the licensee, the children have been enrolled from about 2 months to 2 years. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
