Family home — safe toys and play equipment — Based on observation, the licensee did not comply with the section cited above as LPA observed a swing set in the outside play area which appeared to not be securely anchored to the ground and swayed side to side as well as a slide not in good condition as the slide was cracked and accessible to children in care which poses a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on observation and interview, the licensee did not comply with the section cited above as LPA observed three children in play yards who had blankets while sleeping which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S2 was missing proof of the MMR and Pertussis vaccines which poses a potential health, safety or personal rights risk to persons in care.
