Buildings and Grounds — Based on staff and children interviews, and a review of records from the facility, the facility was not maintaining a comfortable air temperature for children due to a A/C unit in disrepair. This is a potential health and safety and/or personal rights risk to children in care.

Staff immunization requirements — Based on record review on 7/31/24, staff 2, 3 have resigned as of 6/24/24, and 6/27/24, and staff #6 did not have records present at the time of inspection. This poses a potential health, safety and personal rights risk to children in care.

Child Care Center Director Qualifications and Duties — Based on record review on 7/31/24, Director did not have evidence of registration for Preventative Health and safety training which was required by 6/12/24. This poses a potential health, safety and personal rights risk to children in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in out of four out of eight staff do not have current mandated reporter on file at time of inspection. which poses a potential health, safety or personal rights risk to persons in care.

Criminal background check — Based on record review, the licensee did not comply with the section cited above in three out of eight staff are not associated to the facility at time of inspection (Staff #1, 3, 5), which poses an immediate health, safety or personal rights risk to persons in care.
