Teacher-Child Ratio — Upon arrival to the facility, LPA observed one teacher supervising thirteen (13) children with no other staff members present, operating out of ratio. This poses a potential risk to the health and safety of children in care.
Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During the outdoor inspection, Licensing Program Analyst (LPA) observed that on the concrete near where children were playing, there was an accummulation of bird feces on several different areas. LPA also observed that several pieces of playground equipment had areas where the plastic had cracked, leaving sharp/pointed edges. LPA also observed an area where a wooden panel in the wall was splintered and worn. During the indoor inspection, LPA observed that the carpets/rugs in the toddler room had an accummulation of dirt/lint particles, not appearing to have been vacuumed in some time, with some staining. In addition, the colorful carpet in the main room was starting to roll up on the edges, which could be a tripping hazard.
Staff immunization requirements — Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During employee records review, Licensing Program Analyst (LPA) observed that the substitute teacher (staff #2) was missing proof of immunizations (Measles/Pertussis/TB) in her file.
Child Care Center Director Qualifications and Duties — Based on observation and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During employee records review, Licensing Program Analyst observed that a designated substitute director/teacher was not on file as the current Director was not on the premises nor had any paperwork such as the LIC 308 been submitted to the Department with the name of the substitute director or person designated to act on behalf of the Director in her absence.
Notice to parents that site visit reports are available — Based on observation and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During children's records review, Licensing Program Analyst did not observe proof that the facility representative had provided parents with a copy of the licensing report dated 09/05/23, which showed a Type A violation nor was a signed LIC 9224 found in all of the children's files reviewed.
