Reporting Requirements...The items below shall be reported to the Department within 10 working days following their occurrence: (4) Any changes in the plan of operation that affect services to children. Based upon observation, interview, and records, the preschool age children were observed to be using the patio area for outdoor activites which is not a licensed area.
All toilets, handwashing and bathing facilities shall be maintained in safe and sanitary operating condition. Additional equipment, aids and/or conveniences shall be provided as needed in centers that serve children with physical disabilities. Based upon observation, interview, and records the toddler bathroom has not been used in a sanitary or operating condition as it has been used as storage.
Carbon monoxide detectors required — Based on observation, interview and record review, the licensee did not comply with the section cited above as the facility did not have one or more carbon monoxide detectors in the facility which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above as of 2 out of 7 staff did not have current mandated reporter certificates which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above as 2 out of 7 staff did not have proof of immunization records in their staff file which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in 3 of 7 staff are missing TB test and health screening and 2 staff out of 7 are missing LIC 9108, and 3 staff out of 7 are missing LIC 9052, which poses/posed a potential health, safety or personal rights risk to persons in care.
