Criminal Record Clearance — Based on LPA observation, licensee omission, and record review, the licensee did not comply with the section cited above. Beginning November 2023, the licensee allowed one adult to reside in the home, and

Staffing Ratio and Capacity — Based on observation and record review, the licensee did not comply with the section cited above. During the visit, the census is 12 children, 2 infants, 9 preschool age children, and 1 school age child, which poses an immediate health, safety risk to persons in care.

Family home — in-ground pool drowning prevention requirements — Based on observation and interview the licensee did not comply with the section cited above. A pool alarm is present at the facility but not installed or functioning at the moment which poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on observation, interview, and record review, the licensee did not comply with the section cited above. Both Licensee and Staff 1's Mandated Reporter Certificates have expired as of 10/2024, which poses a potential health, safety or personal rights risk to persons in care.

LPAs observed all 3 gallons of milk in the refrigerator were expired as of 08-06-24 & 07-14-24. LPAs also observed multiple “Honest” juice boxes with a “best if used by” date of 04-01-24. Additionally, there was a bag of “LandO’Frost” turkey breast meat slices with a label of “sell by” 08-02-24. LPAs also found mustard with a date of 07/26/2024. — requirement not met

Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above, S1 and S3 immunization records are not on file and not available to view during this visit, which poses a potential health, risk to persons in care.

Family home — home free of safety hazards — Based on record review], the licensee did not comply with the section cited above, C6 Identification and Emergency Information (LIC700), is not on file and available to view during this visit, which poses a potential safety risk to persons in care.

Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above. With licensee's own omission, C3 does not have a physical Infant Sleep Log available for review during the visit which poses a potential safety risk to persons in care.

Application for Initial License Evidence of a current tuberculosis clearance, not more than one year prior to or seven days after initial presence in the home, for any adult in the home during the time that children are under care. — Based on record review, the licensee did not comply with the section cited above, S2 and S3 tuberculosis (TB) clearance is not on file or available to view during this visit, which poses a potential health risk to persons in care.
