Family home — home free of safety hazards — Based on At 10:12am Fire extinguisher was inspected by LPA Calderon and receipt for date of purchase was observed dated 11/05/2024, picture taken , the licensee did not comply with the section cited above in [1] out of [1] fire exting. which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on L CalderonPA reviewed Staff #1 file and they did not have immunization records in place, missing TB, MMR,TDAP and flue (can get or decline), the licensee did not comply with the section cited above in [1] out of [1] no immunization records which poses a potential health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on observation the fire extinguisher was last serviced and inspection tag dated 10/24/2022, the licensee did not comply with the section cited above in [1] out of [1] object-fire extinguisher which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation LPA observed blank 15 min sleep logs for Child #1 and Child #2, Interview with Licensee stated unable to locate sleep logs. Based on record review no logs reviewed for infants, the licensee did not comply with the section cited above in [2] out of [2] infant childre, which poses a potential health, safety or personal rights risk to persons in care.
