Mandated reporter training — During record review and interview with Licensee, Licensee stated she completed mandated reporter training 15 years ago and did not know that she needed to renew it. Licensee's assistant is also missing mandated reported training on file. The licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
