L.Bryant toddlers use the gated area only, construction occurred on 1/17-18/26 & room name changes were made, effective 2/3/26.

In addition, a written report...submitted to the Department within seven days following the occurrence of such event. (B) Any injury to any child that requires medical treatment. The requirement was not met as evidenced by: Facility was informed of injury required medical treatment on 8/13/25 and reporting to the dept. on 8/19/25.
In addition to the sign-in procedure requirement of Section 101226.1(b), the licensee shall develop, maintain, and implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following... The requirement was not met as evidenced by: Case notes on 5/30/25 stated " I signed for the absences for the rest of May since mom never indicated any proof/ signs of disenrollment.' On 7/2/25 LPA received Case Notesfrom Erynn Laurent ?tilte? (EL) , states EL spoke to Staff #1 to inform them on how to properly fill out the sign in and out sheet for children absences. Staff #1 via telephonically on 7/3/25 informed LPA Calderon S1 did a mistake to sign the mothers name indicating their absences.
Napping Equipment — Based on observed in the ' toddler room' children linen on cots, cots stacked on each other bedding (linen) coming to contact with other beddings, the facility represenative did not comply with the section cited above in [10] out of [15] blue cots which poses potential health, safety or personal rights risk to persons in care.
Written directives for drinking water lead testing — record review. Lead test results showed that three water sources had an action level exceedance. LPA observed all three outlets have been been capped off.This is a potential risk to the health and safety of the children in care.
Personal Rights — Based on staff interviews, the licensee did not comply with the section cited above in: staff not addressing teacher’s statement with children present which poses/posed a personal rights risk to persons in care.
Reporting Requirements — Based on record review, the licensee did not comply with the section cited above in: that no telephone or fax report was made to the department within 24 hours of the incident which poses/posed a personal rights risk to persons in care.
