Personal Rights — Based on observation, the licensee did not comply with the section cited above. While touring the outside area of the home at 9:51 a.m. LPA observed dog feces near the childrens play structure and toys which poses an immediate health, safety or personal rights risk to persons in care.

Staff immunization — influenza, pertussis and measles — Based on observation, interview, record review, the licensee did not comply with the section cited above in that, licensee did not maintain immunization record showing proof of pertussis and measles for A1, A2, A3, which poses a potential health, safety or personal rights risk to persons in care.

During staff file reviews, LPA observed licensee and two assistants did not have a current mandated reporter certification. Licensee mandated reporter certification expired 6/25/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Records — Based on observation, interview, and record review, the licensee did not comply with the section cited above. During staff file review, licensee was unable to provide a file for staff#2 (S2) which poses/posed a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on observation, interview, and record review, the licensee did not comply with the section cited above. During children file reviews, LPA observed 5 out 7 children were missing immunization records and licensee could not provide LPA with the records which poses/posed a potential health, safety or personal rights risk to persons in care.

During children file review, LPA observed 2 out of 7 children were missing LIC 995A Notification of Parent Rights which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation and interview, the licensee did not comply with the section cited above. During children file review, LPA observed child #1 (C1) did not have documentation showing licensee was documenting infant safe sleep and licensee was unable to provide proof showing the 15 minute checks during nap times which poses/posed a potential health, safety or personal rights risk to persons in care.
