Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in 1 out of 5 children did not have a completed Physician's report which poses a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — The children's bathroom sink was observed with a lot of build up. Multiple outdoor broken toys including the wooden house that was observed with exposed rotting wood. LPA observed two wooden doors with exposed wood rotting, which posed a potential safety risk to persons in care.
a)The child care center shall be clean,...and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin. This requierments is not met as evidence by: LPA observed the pantry where snacks are stored with visible build up and the toaster in the kitchen area observed with crumbs and build up. The freezer was observed with empty dish containers and plates. The refrigerator was observed with food and stains in the door and bottom of the area, all which pose a potential safety risk to the children in care.
Immunizations — LPA observed 4 out of 10 files missing immunization record which poses a potential risk to the health and safety of the children in care.
Child's Medical Assessments — LPA observed 3 children's file missing a physician's report (LIC 701) which poses a potential risk to health and safety risk to the children in care.
