Responsibility for Providing Care and Supervision — Based on observation, at 10:35AM, a child went to the restroom. There were staff in the room; however, they were not positioned where they can visually supervise the child in the restroom. This poses a potential health and safety risk to children in care.
Buildings and Grounds — Based on observation, and interviews conducted, Licensee did not comply with the section cited above. During investigation, it was discovered there was broken/cracked toys, wood planters that can cause splinters, wooden bench around tree that is broken with nails sticking out and sharp corners, which posed a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — Licensee failed to report an unsual incident report as required that occured on 10/10/2024 were child (C-1) hurt staff (S-1). This poses a potential risk to the health and safety of children in care.
