Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above licensee has mandated reporter training certificate that has expired which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — During visit on 8/28/25, LPAs observed infant child in a play yard. The infant was wedged between the play yard mesh wall and the mattress of the play yard.
Staffing Ratio and Capacity — During visit on 8/28/25, LPAs observed five (5) infant children under the age of 24 months in care during normal childcare operating hours being cared for and supervised by licensee and fingerprint cleared staff.
Family home — home free of safety hazards — LPA observed detergents, cleaning solutions, disinfectants, and health & hygiene, and beauty supplies in the laundry room, on kitchen counter, and in bathroom on sink and in cabinets where they are accessible to children in care.
Infant Safe Sleep — LPA requested infant sleep chart. Infant sleep charts not completed as the infant sleeps in real time. Per assistant, infant sleep chart is completed at the end of each week and stored on computer. LPA observed sleeping infant at 1:03p, 1:18p, 1:33pm. No sleep chart documentation completed.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 2 out of 4 employees are missing mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review the licensee did not comply with the section cited above in 4 out of 4 employees are missing immunization on file which poses a potential health, safety or personal rights risk to persons in care.
