Materials provided to an applicant at orientation — Based on observations and interviews, the licensee did not comply with the section cited above. During the inspection, the licensee stated that her husband was present and acting as her assistant. LPA attempted to locate the husband in the areas identified by the licensee, but he was not present. Approximately 5–7 minutes later, LPA observed the husband entering the home from the garage stairs. Providing false information to the Department during an inspection interferes with Licensing’s ability to assess staffing, supervision, and capacity, and poses a potential risk to the health and safety of children in care.
Mandated reporter training — Based on the record review, the Licensee did not comply with the section cited above. Licensee and her helpers have not completed their Mandated reporter training, which poses/posed a potential health, safety, or personal rights risk to persons in care.
Alterations to Existing Building or Grounds — Based on observation, interview, and record review, the licensee utilized the kitchen without prior approval from the Licensing (off-limit area). This poses a potential health and safety hazard to children in care.
