Staffing Ratio and Capacity — During the time of arrival, LPA noted the assistant parking her car, then getting out of her car and go into the home. Upon arrival, licensee was not home, a home resident was, who was caring for 10 children while licensee and assistant were out of the home. Furthermore, home resident did not meet all required documentation. This poses an immediate health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on LPA observation, the sheet the infant was sleeping on was not a fitted sheet. Although this typically poses an immediate threat to the health and safety of children, the infant is 19 months, downgrading this to a Type B.This poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review and licensees own admission, the licensee did not comply with the section cited above. Per licensee, although S2 does not always provide care for children in care and/or assist, S2 was left alone with 10 children in care. S2 does not have an updated Mandated Reporter Training (MRT) Certificate for review. This poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review and licensees own admission, the licensee did not comply with the section cited above. S2 did not have immunizations (measles(MMR), pertussis(Tdap)) on file. This poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above. 1 out of 2 staff did not have an updated Mandated Reporter Training (MRT) on file. Although licensee was able to provide proof of "general" training, the one for "Child Care Providers" has expired. This poses/posed a potential health, safety or personal rights risk to persons in care.
