Criminal Record Clearance — Based on interview, the licensee did not comply with regulation as a disabled, adult male receiving care from the licensee and serving as volunteer support to his enrolled sibling, is an uncleared adult, which poses an immediate risk to the health, safety and personal rights to persons in care.

Infant Safe Sleep — Based on LPA observation, a blanket, a stuffed animal, and a baby bottle were inside a child’s crib while they were inside, which poses a potential health, safety or personal rights risk to persons in care.

Child's Records — Based on record review and interview, the licensee did not comply with the section cited above in the case of 5 of 11 children in care did not have verification of enrollment, which poses a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on record review and interview, the licensee did not comply with the section cited above in the case of 4 of 11 children in care. Not maintaining immunization records for children poses a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on interview and record review, the licensee did not comply with the section cited above, as she did not possess and consistently maintain the safe sleep logs for 3 of 3 infants in care, which poses a potential health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on LPA observation there were loose closures to latches on kitchen cabinets and missing latches on the bathroom’s under sink cabinet where cleansers were stored, which poses a potential health, safety or personal rights risk to persons in care.

Family home — admission procedures and parent rights — Based on interview, the licensee did not comply with the section cited above, as 2 of 2 children enrolled did not have paper work, which poses a potential health, safety or personal rights risk to persons in care.
